NOTICE OF NONDISCRIMINATION
Hudson Facial Plastic Surgery (“Practice,” “we,” “us,” or “our”), located at 1640 N Wells St. Unit 207, Chicago, IL 60614, complies with applicable federal and Illinois civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, sex, sexual orientation, gender identity, marital status, religion, or any other characteristic protected by applicable law in the administration of its programs, services, or activities.
Hudson Facial Plastic Surgery:
- Does not exclude people or treat them differently because of race, color, national origin, age, disability, sex, sexual orientation, gender identity, marital status, or religion
- Provides free aids and services to people with disabilities to communicate effectively with us, including qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats, and other formats upon request)
- Provides free language assistance services to people whose primary language is not English, including qualified interpreters and written information translated into other languages upon request
1. LEGAL BASIS FOR NONDISCRIMINATION
Our commitment to nondiscrimination is grounded in the following federal and Illinois laws:
1.1 Federal Law
- Section 1557 of the Affordable Care Act (ACA), 42 U.S.C. § 18116: Prohibits discrimination in covered health programs and activities on the basis of race, color, national origin, sex (including pregnancy, sexual orientation, and gender identity), age, and disability
- Title VI of the Civil Rights Act of 1964, 42 U.S.C. § 2000d: Prohibits discrimination on the basis of race, color, and national origin in programs receiving federal financial assistance
- Title II of the Americans with Disabilities Act of 1990 (ADA), 42 U.S.C. § 12131: Prohibits discrimination on the basis of disability in programs, services, and activities provided by public entities; Section 504 of the Rehabilitation Act of 1973 similarly prohibits discrimination on the basis of disability in programs receiving federal financial assistance
- The Age Discrimination Act of 1975, 42 U.S.C. § 6101: Prohibits discrimination on the basis of age in programs receiving federal financial assistance
- Title III of the ADA, 42 U.S.C. § 12181: Requires places of public accommodation, including medical offices, to provide equal access to individuals with disabilities
1.2 Illinois Law
- Illinois Human Rights Act (IHRA), 775 ILCS 5: Prohibits discrimination in public accommodations and services on the basis of race, color, religion, national origin, ancestry, age, sex, marital status, order of protection status, disability, military status, sexual orientation, pregnancy, and unfavorable discharge from military service
- Illinois Accessibility Code (71 Ill. Adm. Code 400): Establishes accessibility standards for buildings and facilities in Illinois, including medical offices
- Illinois Health Care Right of Conscience Act (745 ILCS 70): Protects the rights of patients and healthcare providers in certain circumstances
- Illinois Equal Pay Act (820 ILCS 112): Referenced for completeness as applicable to our employment practices

4. CHICAGO AND COOK COUNTY NONDISCRIMINATION REQUIREMENTS
In addition to federal and Illinois state law, Hudson Facial Plastic Surgery complies with applicable City of Chicago and Cook County human rights ordinances, including:
- Chicago Human Rights Ordinance (Chicago Municipal Code § 2-160): Prohibits discrimination in public accommodations on the basis of race, color, sex, gender identity, age, religion, disability, national origin, ancestry, sexual orientation, marital status, parental status, military discharge status, and source of income
- Chicago Fair Housing Ordinance: Referenced for completeness regarding our non-discrimination commitment
- Cook County Human Rights Ordinance: Prohibits discrimination on the basis of protected characteristics in unincorporated Cook County
5. HOW TO FILE A DISCRIMINATION COMPLAINT
If you believe that Hudson Facial Plastic Surgery has failed to provide these services or discriminated against you in another way on the basis of race, color, national origin, age, disability, sex, sexual orientation, or gender identity, you have the right to file a grievance or complaint. You will not be retaliated against for filing a complaint.
